Zorgvignetten bij patiënten met Ernstige Psychiatrische Aandoeningen nader onderzocht met data uit meerdere patiëntgebonden regionale registratiebronnen

De langdurende GGZ wacht grote veranderingen zoals verdergaande ambulantisering (de beddenafbouw met 30% tot 2018) en de transitie van de langdurige zorg (een deel van de patiënten van de FACT-teams gaat hulp in de 1e lijn ontvangen; de RIBW’s gaan onder verantwoordelijkheid van de gemeente vallen). Dit heeft veel gevolgen voor de organisatie van de zorg. Om hulpverlening aan patiënten met een ernstig psychiatrische aandoening (EPA) goed te doen is het belangrijk tijdig te onderkennen waar er knelpunten optreden in de keten. Vier zorgaanbieders in de regio Utrecht (Altrecht, Kwintes, SBWU en Victas) hebben een aanpak bedacht, waarmee zij goed zicht krijgen op deze doelgroep. De vier instellingen hebben, in samenwerking met KPMG Plexus, vignetten ontwikkeld voor EPA-cliënten. De indeling van patiënten in zorgvignetten wordt door beleidsmakers (GGZ gemeenten, ziektekosten-verzekeraars) gezien als een hulpmiddel om de doelgroep onder te verdelen in specifiekere groepen met een verschillende zorgzwaarte. Dit biedt de ketenpartners inzicht in de omvang van zorgvragen en biedt een gemeenschappelijke taal om samen afspraken te maken over welke zorg er door wie in de regio geleverd moet gaan worden. De zorgzwaartevignetten zijn gebaseerd op een ‘historische foto’ van het zorggebruik door EPA-patiënten in de regio Utrecht in 2012.

The overall number of involuntary admissions is increasing in many European countries. Patients with severe mental illnesses more often progress to stages in which acute, coercive treatment is warranted. The number of studies that have examined this development and possible consequences in terms of optimizing health care delivery in emergency psychiatry is small and have a number of methodological shortcomings. The current study seeks to examine factors associated with compulsory admissions in the Amsterdam region, taking into account a comprehensive model with four groups of predictors: patient vulnerability, social support, responsiveness of the health care system and treatment adherence.

Cost-effectiveness of preventing depression in primary care patients: randomised trial.

Little is known about the cost-effectiveness of preventing mental disorders. An economic evaluation was conducted alongside a randomised clinical trial. Primary care patients with sub-threshold depression were assigned to minimal contact psychotherapy plus usual care (n=107) or to usual care alone (n=109).

Opportunities for cost-effective prevention of late-life depression: an epidemiological approach.

Clinically relevant late-life depression has a prevalence of 16% and is associated with substantial societal costs through its disease burden and unfavorable prognosis. From the public health perspective, depression prevention may be an attractive, if not imperative, means to generate health gains and reduce future costs.
